Discrimination, Harassment, Sexual Misconduct, or Retaliation

Reports and claims submitted via the Sexual Misconduct form above will be submitted to the Title IX Office. Please contact the Title IX Office at (217) 333-3333 or see wecare.illinois.edu for assistance with the form, if you prefer not to use the online reporting form, or have other questions about sexual misconduct resources.
Complaints or grievances alleging discrimination on the basis of disability and/or potential violations of the Americans with Disabilities Act or the Rehabilitation Act will be addressed in consultation with the ADA Coordinator.
Request an Accommodation

Report a Barrier to Access
If you encounter conditions that make it difficult for people with disabilities to access university facilities, programs or services, please use our Report A Barrier form to let us know.
Examples include a blocked access ramp or accessible parking space, motorized door opener that isn’t working, inaccessible websites, videos without captions, or an inability to access an event or program. Reports are received by the ADA Coordinator and addressed in collaboration with relevant campus units.
