Reporting Options and Resources
The Office for Access & Equity is your point of contact to report several different kinds of concerns and get help. Please scroll down or use the menu to the left to learn more.
If there is an immediate risk to health or safety, please call 911 to connect with medical support or local law enforcement. Reports submitted using these forms may not be reviewed outside normal business hours.
Report a Barrier to Access
If you encounter conditions that make it difficult for people with disabilities to access university facilities, programs or services, please use our Report A Barrier form to let us know. This could include inaccessible websites, videos without captions, a blocked access ramp, an inactive motorized door, or an inability to access an event or program.
Request an Accommodation
Employees who have a disability, including faculty and staff as well as graduate and undergraduate students who work on campus, as well as job applicants, can request reasonable accommodations that will enable them to have equal access to employment opportunities at Illinois. Employees with needs related to their religious beliefs and practices, or pregnancy, childbirth, or related conditions, can also request an accommodation.
Discrimination, Harassment, Sex-Based Misconduct, or Retaliation
Reports and claims submitted via the sex-based misconduct form above will be submitted to the Title IX Office. If you prefer not to use the online reporting form, need assistance in completing the forms, or encounter accessibility problems with the forms, please contact the Title IX Office at (217) 333-3333 or browse to wecare.illinois.edu for assistance.
Complaints or grievances under the Americans with Disabilities Act or the Rehabilitation Act will be shared with, and addressed in consultation with, the ADA Coordinator.
